Has anyone ever toured on Mex 45 or know anything about this road?
Level of difficult, any potential dangers, etc...
any information at all.

Quieter routes might be better
On one trip into Mexico we went Nuevo Casas Grandes south toward Creel - and that route is delightful. Quiet, cool. Low traffic.
On our recent huge trip through the Americas we came out of Copper Canyon and south of Hidalgo de Parral we followed that delightful tiny road south to Durango. Not much to see, but the road was fine. I would think much better than the big road.
